Localized tenosynovial giant cell tumor

Diagnosis almost certain

Presentation

The patient presents with a painless mass of the left middle finger.

Patient Data

Age: 50 years
Gender: Female

There is focal, periarticular soft tissue swelling of the left middle finger. This is specifically centered at the DIP and terminal phalanx, medial and volar soft tissues. There is pressure erosion of the medial cortex of the terminal phalanx, especially when compared to the other digits. There is no associated calcification or extra-skeletal ossification.

ultrasound

A solitary, lobulated, hypoechoic, soft tissue mass lesion is confirmed at the level of the left 3rd DIP and terminal phalanx, volar soft tissues, abutting the flexor tendon. There is non-aggressive peripheral vascularity.

Case Discussion

Features suggestive of a localized tenosynovial giant cell tumor with a typical plain film and sonographic appearance and involving a typical site (left 3rd digit). Excision histology was recommended.
