Locally advanced gallbladder adenocarcinoma

Diagnosis almost certain

Presentation

Right upper quadrant pain and jaundice.

Patient Data

Age: 55 years
Gender: Male

A large 117× 88 mm hetero-enhancing mass is noted at the anatomical location of the gallbladder that infiltrates the adjacent liver, the first part of the duodenum and the hepatic flexure of the colon. Surrounding fat stranding and several regional enlarged lymph nodes are also observed. Intra and extra-hepatic bile ducts are dilated, and CBD is measured 12 mm in caliber.

A few small peritoneal soft tissue density masses of less than 20 mm also suggest tumoral deposits.

A few non-enhanced simple cortical cysts are seen in both kidneys.

The prostate gland is enlarged.

Degenerative changes such as osteophytosis are seen in the lumbar spine.
Evidence of CD insertion is present at the lower lumbar vertebrae.

Pathology-proven gallbladder adenocarcinoma with local invasion into the adjacent liver, duodenum and colon; regional lymphadenopathy and peritoneal tumoral deposits.
