Presentation
Posterior thigh mass.
Patient Data

There is a large fat-intensity mass within the posterior compartment of the thigh measuring approximately 6.2 x 6.1 x 20.1cm (AP x ML x SI). It appears to be deep to the fascia and within the posterior compartment of the thigh rather than subcutaneous in location. The lesion is predominantly fat intensity signal. Internally, there are thin internal septations that are enhancing that are thicker inferiorly. Inferiorly there is minimal nodular enhancement. The margins appear smooth and well defined. There appears to be no invasion.
Case Discussion
This mass was operated on with histology revealing a low-grade liposacroma. On imaging, the mass has mostly benign lipoma-type features except on the post contrast sequences where there are relatively thick enhancing septations and on the coronal the impression of some nodularity.
