Fever and chills with right mandibular swelling

Age: 65 Years
Gender: Male

There is soft tissue gas and edema is present in the submandibular region. In the right parapharyngeal space, there is a multiloculated fluid collection measuring at least 3.4 x 2.9 x 5.1 cm. There is amorphous fluid along the right parotid gland which could represent a developing abscess and a collection of fluid along the right jaw superficially measuring approximately 3.9 x 3.9 x 1.3 cm, also compatible with abscess. Additionally, there is a collection of gas and fluid along the floor of the mouth laterally on the left measuring approximately 2.7 x 1.5 x 3.1 cm. Post- operative images

Since the prior study 4 days ago, tracheostomy tube has been placed. The fluid collection at the right base of tongue is decreased and the residual right paralaryngeal fluid is slightly smaller with residual compression of the supraglottic airway. A submental fluid collection with air-fluid level is increased in size measuring 2.2 x 4.0 cm in transverse plane. A left submandibular fluid collection is increased in size measuring 4.0 x 2.7 cm in transverse plane. Diffuse subcutaneous edema and scattered gas bubbles in the right lateral neck have increased and irregular fluid collections persist in both submandibular glands. There is subcutaneous edema and gas tracks to the right supraclavicular soft tissues. The lower molars appear impacted bilaterally and a right upper molar demonstrates no significant decay and periapical lucency.

Case Discussion

This case demonstrates Ludwig angina, a rapidly progressive cellulitis of the submandibular space, usually due to dental infections; treatment involves IV antibiotics and removal of the inciting tooth.
