Lumbar spine schwannoma - recurrent

Presentation

History of schwannoma resected one year ago. She is currently attending due to pain in the lumbar region and weakness in the right lower extremity.

Patient Data

Age: 50 years
Gender: Female

On the present MRI study at L2-L3 level, at right side an extramedullary lesion is observed, isointense to the spinal cord parenchyma can be observed on T1 and hyperintense on T2 and with a heterogeneous appearance that, when using contrast medium, shows equally heterogeneous enhancement. Osteodegenerative changes are also documented in Modic type II vertebral bodies at L1-L3 levels, best demonstrated in fat-sat sequences with very hypointense areas.

Case Discussion

Given the previous history of schwannoma and due to the characteristics of the lesion, it is likely that we are dealing with a recurrent schwannoma. No current histopathological studies are available. Previous MRIs were not performed at this hospital

Schwannomas are benign tumors derived from schwann cells, which produce myelin in peripheral nerves, they cover 25-30% of spinal masses and their recurrence after post-surgical treatment is less than 5%.
