Lumbar vertebral aneurysmal bone cyst

Diagnosis almost certain

Presentation

Tender lumbar spine following a fall. X-ray to rule out a fracture.

Patient Data

Age: 17 years
Gender: Female

There is an expansile, bubbly lesion seen arising from the posterior elements on the left side of L3. The cortex is intact and the lesion demonstrates internal lumen septae. The lesion is benign in appearance, consistent with an aneurysmal bone cyst. The differential diagnosis includes osteoblastoma and osteochondroma, however these are less likely.

Case Discussion

Imaging is almost pathognomonic but it is conceivable that an unusual appearing other entity could have similar appearances. Imaging carried out three years later show no change, reinforcing this point.
