Lung cancer and adrenal mass CT-guided biopsy

Presentation

Smoker with a lung mass in the right lower pulmonary lobe and a huge mass in the left adrenal. The patient was submitted to a previous core-needle biopsy with inconclusive results. Was referenced to a new diagnostic procedure.

Patient Data

Age: 70 years
Gender: Male

The patient had previous CT exams in another hospital, on the CT scout for planning the biopsies is possible to identify the masses on right lower lobe and on left adrenal (arrows). 

Two different sites were approached on this procedure: first a core-needle was introduced to achieve left adrenal mass by a left paravertebral path; after that another core-needle was introduced by an intercostal path to achieve the right basal pulmonary mass. 

Case Discussion

After the case discussion with the oncologist, as the patient was submitted for two previous core-needle biopsies in other service and there is a doubt about the origin of the tumor, we opted for both masses approach. 

As pulmonary mass biopsy could be complicated with a pneumothorax, adrenal mass biopsy was performed first. The procedure had no complications.

Pathological examination revealed lung cancer with adrenal metastasis
