Lymphangioma

Presentation

Swelling right side of tongue

Patient Data

Age: 30 years
Gender: Female
mri

Ill-defined multi-cystic, multiseptated altered signal intensity lesion measuring approx. 50 x 27 x 52 mm involving right half of tongue.

The lesion is involving anterior 2/3rd as well as posterior 1/3rd of tongue with extension across the mid-line by about 3.5 mm. 

The lesion is extending up to floor of mouth, splaying and possibly involving mylohyoid muscle.

It involves the sublingual space, extending to submandibular space and involving right submandibular gland. It is reaching up to soft palate superiorly with no evidence of infiltration. 

Mass effect is seen on oropharyngeal airway. 

It appears heterogeneously hyperintense on T2/STIR images with multiple hypointensities - likely representing calcifications. A few blood-fluid levels also seen which are better appreciated on sagittal images. 

 

Case Discussion

The appearance of the lesion and the involvement of multiple neck spaces are suggestive of lymphangioma.
