Lymphocytic interstitial pneumonitis

Case contributed by Craig Hacking
Diagnosis almost certain

Presentation

Shortness of breath on exertion and dry cough for several years. Non smoker.

Patient Data

Age: 60 years
Gender: Female

There are numerous thin-walled well-defined smooth cysts of various sizes randomly distributed throughout the lungs. Scattered ground glass attenuation and small nodules are also seen. These appearances are stable from previous CTs in the last 2 years.

No pleural effusion or lymphadenopathy. Coronary arterial calcification and LAD stent noted.

Case Discussion

Findings are typical for lymphocytic interstitial pneumonitis (LIP) with diffferentials of LCH (not bizarre cysts, not sparing costophrenic angles and non smoker) and LAM (too old).

The patient had a history of Sjogren's / Sicca syndrome, which has a known association with LIP.
