Malignant giant cell tumor

Case contributed by Dr Utkarsh Kabra

Presentation

Pain in left cervicodorsal region.

Patient Data

Age: 50 years
Gender: Male
MRI

Heterogeneous expansile lesion involving left posterior third rib with areas of cortical breach. The lesion is markedly hyperintense to muscle on fluid-sensitive sequences (here exemplified by STIR) and isointense to muscle on T1-weighted sequence.

The lesion is displacing adjacent left lung parenchyma anteriorly, abutting left lateral aspects of D3 vertebral body medially, left D3 transverse process posteriorly and displacing intercostal muscles.

It is abutting lateral aspects of left D3-4 neural foramina with no obvious intraforaminal extension.

It measures approximately 54 x 68 x 60 mm (AP x TD x CC) and extends from D1-D2 to D3-D4 levels.

Case Discussion

Imaging findings in a biopsy-proven case of malignant giant cell tumor of bone. MRI was done to look for any neural involvement.
