There is grey-white matter differentiation loss within the left MCA vascular territory. Loss of the insular ribbon sign is noted.
Bilateral symmetric glopus pallidus calcifications.
The patient developed a hemorrhagic transformation occupying the left cerebral hemisphere, with intraventricular extension filling all ventricular system and marked perifocal edema. Marked midline shift to right and left uncal herniation are noted.
Contralateral dilatation of right lateral ventricle denoting obstructive hydrocephalus due to compression upon the third ventricle.
Effacement of gyri and obliteration of brain sulci, in keeping with raised intracranial pressure.

Malignant middle cerebral artery (MCA) infarction refers top rapid neurological deterioration of infarction due to cerebral edema or hemorrhage following large MCA infarction.
The patient passed away.
