Malignant retroperitoneal fibrosis

Diagnosis almost certain


Admitted for the purpose of surgical treatment of hernia. During surgery, 3 liters of fluid was removed and the surgeon saw a retroperitoneal mass of fibrous tissue.

Patient Data

Age: 80 years
Gender: Female

Mantle of soft tissue encansing aorta, inferior vena cava and ureters, with caudal extenson to the iliac vessels. The right kidney is also encased, with right hydronephrosis. Avid contrast enhancement. 

Case Discussion

The findings are consistent with "malignant" retroperitoneal fibrosis, and must be differentiated from retroperitoneal metastasis, lymphoma, aortitis, and retroperitoneal hemorrhage.
