Malrotation and volvulus

Presentation

Term baby. Well. Bilious vomit.

Patient Data

Age: 4 days
Gender: Female
Fluoroscopy

Multiple single fluoro-grabs and then a fluoro-run at the end.

Contrast exits the stomach as expected. Duodenum is initially normal. However, the DJ flexure is low and the distal duodenum is dilated.

There is no swirl sign and contrast does enter the non-dilated jejunum.

Case Discussion

The clinical and radiological concern here was for malrotation and volvulus. The patient was taken to theater and volvulus secondary to malrotation was confirmed.

This case illustrates that even in cases of volvulus contrast can still sometimes pass distally.
