Gastric distension with gas. Gas distally in left flank.
Appearances highly suspicious of volvulus given the history.

The cine demonstrates swirling of the small bowel mesentery that involves the SMA and SMV confirming midgut volvulus. There is Doppler signal within the vessels. Trace free fluid, but no other abnormality.
Case Discussion
Given confirmation of small bowel volvulus on the ultrasound, the pediatric surgeons took the patient to theater.
The midgut volvulus was confirmed and de-rotated. Malrotation was also confirmed and a Ladd's procedure performed.
