Presentation

Work up for abdominopelvic pain.

Patient Data

Age: 50 years
Gender: Female

A 55×40 mm well defined mass with fat fluid level, a small focus of calcification, and Rokitansky nodule are present at left adnexa compatible with mature cystic ovarian teratoma.

A peripheral, dense, spotty enhancing mass, approximately 80×45 mm in diameters, is seen at the VII hepatic segment (Couinaud`s nomenclature) on the early post contrast phase. In the equilibrium phase, the attenuated areas spread through the mass and fill in. The density of enhancing components is almost the same as that of the aorta in all phases of the CT scan. Additionally, four small masses less than 20 mm, with a similar enhancement pattern are seen at the right liver lobe in the vicinity of the mentioned mass. Features are consistent with hemangiomas.

A few nonenhanced simple cortical cysts are seen in both kidneys.

Case Discussion

Features on CT scan are typical for right mature cystic ovarian teratoma, which is the most common ovarian neoplasm. These slow-growing tumors contain elements from multiple germ cell layers.
