Mature cystic ovarian teratoma

Diagnosis almost certain

Presentation

Work up for pelvic pain and palpable mass on physical exam.

Patient Data

Age: 40 years
Gender: Female

A 105×65 mm well defined mass with the internal fat component, focal calcifications, and Rokitansky nodules is present at the right adnexa. Eccentric soft tissue component is also observed at the medial aspect.

A few small cysts are seen in the liver less than 8 mm.

Case Discussion

The patient underwent right adnexal mass resection, and histopathology evaluation confirmed mature cystic ovarian teratoma without malignant transformation. 

In the cases of mature cystic ovarian teratoma, whenever the size exceeds 10 cm or soft tissue plugs and cauliflower appearance with irregular borders are seen, malignant transformation should be suspected.
