Presentation

Pelvic pain.

Patient Data

Age: 25 years
Gender: Female
x-ray

Calcific density at the right side of the pelvis.

Right ovarian complex cyst with a dependent fat-fluid level with the upper part of fat signal (high signal on T1 and T2) confirmed by complete suppression of signal on fat sat sequence (STIR) and lower part heterogeneous signal of mainly T1 hypointense and T2 hyperintense signal. The lower part shows a rod-shaped structure of a hypointense signal in all sequences matching with the radiographically present calcification at right side of the pelvis.

Case Discussion

The presence of calcification and fat content in an ovarian cyst is pathognomonic for mature cystic teratoma. It is the most common ovarian mass, usually diagnosed in middle-aged women. It is slowly growing and annual follow-up is recommended for small-sized cysts.
