Non-contrast CT there is a well-defined oval hyperdense lesion noted near the right insula along the expected course of the MCA.

On post-contrast imaging, there is saccular aneurysmal dilatation in the M1 segment of the right MCA/ICA bifurcation.

The anterior cerebral arteries, left middle cerebral artery, posterior cerebral arteries, and vertebral and intracranial internal carotid arteries are patent and of normal calibre, without stenosis or aneurysmal dilatation.

Case Discussion

Unruptured cerebral aneurysms can be found incidentally on brain imaging to evaluate other causes as seen in this patient.
