Case contributed by Piyush P Siwach
Diagnosis certain

Presentation

Known skeletal dysplasia. Recent onset left chest pain and tenderness.

Patient Data

Age: 25 years
Gender: Male
x-ray

Extensive expansile lytic rib lesions bilaterally, many showing ground glass matrix. Many ribs show deformities, suggesting past pathological fractures. There is probably an acute fracture in left eighth rib, the likely cause of left chest pain.

Case Discussion

This patient had already been diagnosed with McCune-Albright syndrome, and presented on this occasion with new-onset left chest wall pain. The chest and rib X ray series demonstrate extensive polyostotic fibrous dysplasia in the ribs bilaterally. The patient also had additional multiple lesions in long bones, not included here.
