Meckel diverticulum involved in Crohn disease

Diagnosis probable

Presentation

Intermittent abdominal pain mainly in the right iliac fossa.

Patient Data

Age: 40 years

The CT images reveal a thick, enhancing walled of the terminal ileum (length = 21 cm) with submucosal fatty infiltration "fat halo sign".

There is a blind-ending tubular structure adjacent to the terminal ileum, originating from the anti-mesenteric border of another ileal loop with thick, enhancing walled as well as "fat halo sign", indicating Meckel diverticulum most likely involved in Crohn disease.

The bone window shows the extraintestinal manifestations of Crohn disease involving the spine and joints. 

Case Discussion

The diagnosis of Crohn disease was confirmed by upper GI endoscopy with ileal biopsy. A Meckel diverticulum, if a present, may or may not be involved in Crohn's disease.
