Meconium plug syndrome

Presentation

Term baby. Failure to pass meconium. Abdominal distension.

Patient Data

Age: 1 day
Gender: Male
x-ray

Multiple loops of gas-filled bowel. No particularly distended bowel loops. NG tube in the stomach. Rectal tube in situ.

Fluoroscopy

Initial contrast flows into a normal rectum and sigmoid. Some filling defects in descending colon with a slight reduction in caliber.

Multiple filling defects throughout the large bowel.

Case Discussion

In this case of failure to pass meconium, the colon is of normal caliber and contains meconium plugs that have caused the obstruction as meconium plug syndrome.

The study confirms the diagnosis but is also therapeutic, allowing repeated injection of contrast until the filling defects have been flushed out.

The baby made an uncomplicated recovery and was discharged a few days later.
