Oblique fracture of the medial malleolus with intra-articular extension and minimal displacement.

Case Discussion

CT study shows isolated oblique medial malleolus fracture. It can be chip fracture, transverse fracture, oblique fracture, vertical fracture, or comminuted fracture.

Oblique medial malleolar fracture is reflecting an unstable ankle. Surgery is often recommended rather than casting, with screws fixation (ORIF).

As for this limited ankle CT study, full leg imaging is recommended to exclude the possibility of associated proximal fibular fracture (Maisonneuve fracture).
