Medial orbital wall and orbital floor blow-out fracture

Presentation

High force blow to right side orbit with sudden swelling of right side eyelids and limitation of right eye movement.

Patient Data

Age: 30 years
Gender: Female

Blow-out fractures in right orbit medial wall extended within related right side ethmoid roof and orbit floor with orbital fat and medial rectus muscle entrapment with intraconal and extraconal and eyelids preseptal emphysema are seen. Note also submucosal hemorrhage in right side maxillary and ethmoid sinuses.

Case Discussion

The case illustrates the medial rectus muscle herniation within the right orbit medial wall blow-out fracture that can lead to muscle necrosis and also the extension of the fracture within the related right ethmoid roof that can lead to CSF fistula and leakage as a major complication of orbital medial wall blow-out fracture.
