Blowout fracture of the medial wall of the left orbit with displacement of the medial rectus muscle into the ethmoid air cells. The globe is intact.

Case Discussion

Although the medial rectus muscle is displaced medially into the ethmoid air cells, muscle entrapment is a clinical diagnosis. Inferior wall fractures are the most common blowout fractures while medial wall fractures are second most common.
