Temporal lobe epilepsy.
Patient Data
Irregular well-corticated defects in the floor of the middle cranial fossa is noted on the left with distortion of the adjacent temporal lobe. Similar, but less marked, right sided changes.
A lobulated cystic abnormality at the left middle cranial fossa anteriorly with distortion of the temporal pole is associated with bony defect and extension of part of the brain into the defect. A similar but smaller defect is present on the other side.
Features are those of bilateral, left greater than right, sphenoid meningoencephaloceles.
Case Discussion
Sphenoid wing encephaloceles are increasingly recognized in the setting of intracranial hypertension. No compatible history, other than demographics, is present in this case.
