Mediastinal abscess

Presentation

History of abscess formation in retropharyngeal space. The abscess drained a week ago. Now pus and secretion from the excisional part of the abscess in the lateral part of the neck.

Patient Data

Age: 20 years
Gender: Male

Evidence of air tracking in the retropharyngeal space, extending to the posterior mediastinum with a large rim enhancing collection containing specks of air, lateral to the esophagus, posterior to the right main bronchus, and right brachiocephalic vein.
A tract is noted in the left anterior neck containing air specks, communicating with the large collection.
Surgical emphysema is noted in the posterior lower neck spaces.
Patchy ground-glass opacities are noted in the upper lobes of both lungs, more pronounced on the right side. Lung emphysematous changes also in the anterobasal segments of both lower lobes.

Features representing mediastinal abscess formation, a known complication of retropharyngeal abscess.
