Mediastinal pancreatic pseudocyst

Presentation

Chronic nonspecific abdominal pain

Patient Data

Age: 15 years
Gender: Female

CT changes of chronic pancreatitis with significant pancreatic atrophy.  Multiple foci of dystrophic calcification in the head of the pancreas.

Large well-defined elongated cystic lesion, seen arising from the body of the pancreas and extending through the diaphragmatic hiatus into the posterior mediastinum causing anterior displacement of the esophagus.

Incidental finding of an atrophied left kidney.

Case Discussion

The CT changes of chronic pancreatitis along with large epigastric/mediastinal cystic lesion makes the diagnosis of pancreatic pseudocyst the most likely.

The cyst is interestingly seen extending through the esophageal hiatus into the posterior mediastinum.
