Presentation

Severe headache and vomiting.

Age: 17 years
Gender: Female

CT brain

There is a large posterior fossa hyperdense midline lesion. No calcification or hemorrhage. Marked hydrocephalus is evident with transependymal edema. 

MRI brain

MRI confirms the presence of a midline lesion, which appears to arise from the vermis. The lesion is of low signal intensity on T1 and intermediate signal intensity on T2 weighted images. High signal intensity is noted on DWI. Heterogeneous enhancement is seen on post contrast sequences. 

Acute hydrocephalus with transependymal edema is seen as well.

Case Discussion

This patient went on to have a posterior fossa craniotomy and resection. The mass was proven to be a medullobastoma.
