Mega cisterna magna

Case contributed by Ali Abdullah Obaid
Diagnosis certain

Presentation

Workup for headache and to check for any anomalies.

Patient Data

Age: 8 years
Gender: Male
ct

Focal enlargement of the CSF-filled subarachnoid space in the inferior and posterior portions of the posterior cranial fossa, no mass effect, and normal crossing subarachnoid fold through retro cerebellar space.

Normal both cerebellar hemispheres and cerebellar vermis.

Normal both lateral ventricles with no evidence of hydrocephalus. Normal 4th ventricle.

Case Discussion

Mega cisterna magna is an incidental finding on neuroimaging, and no imaging follow-up is necessary. The presence of a normal posterior fossa and vermis distinguishes it from a Dandy-Walker malformation, and the absence of mass effect distinguishes it from the arachnoid cyst.
