There is an extra-axial dural-based lesion (10x8x7 mm) of the left cerebellopontine angle, anterior to the internal auditory canal (IAC) of intermediate signal on T1WI, low signal on T2WI, and high signal on FLAIR. The postcontrast sequences show an intense and homogeneous enhancement with mild extension into the IAC. No significant mass effect on the ipsilateral VII and VIII cranial nerves.
Case Discussion
MRI features are most consistent of a CPA meningioma. The main differential diagnosis is the acoustic schwannoma, which usually has a high signal on T2 with central cystic changes as well as the intracanalicular extension which is a constant feature.
