Meningioma - parietal convexity

Case contributed by Ammar Haouimi
Diagnosis almost certain

Presentation

Headaches.

Patient Data

Age: 45 years
Gender: Female

Well-circumscribed extra-axial mass of left parasagittal location with a broad-based dural attachment to the parietal convexity, extending to the skull vault. It appears isointense to the cortical grey matter on T1, T2, and FLAIR with central areas of low signal and mild surrounding vasogenic edema. The postcontrast sequences show intense and heterogeneous enhancement with central areas of cystic changes and dural tail signs well-visualized on coronal and sagittal postcontrast sequences. No evidence of superior sagittal sinus invasion.

A thin rim of high signal around the mass on coronal T2 "CSF cleft sign", indicating the extra-axial location of the mass.

Partially empty sella is noted

Case Discussion

MRI features of meningioma of the falx cerebri.

The patient went on to have complete resection of the tumor with a histopathological exam that confirmed the diagnosis of a transitional meningioma grade 1.

Transitional meningiomas are also known as mixed meningiomas because they have components of meningothelial and fibrous subtypes of meningiomas.
