Plain images show focal peripheral hypodensity in right parieto-occipital white matter.
Post-contrast images show dural based enhancing lesion.
Conclusion: Meningioma in right parieto-occipital region with surrounding edema.
Case Discussion
The hypodensity in plain images may be challenging. It does not always mean ischemia, hence the clinical presentation, the features of hypodensity (non-territorial, nor typical for deep white matter locations) and further imaging correlation needs to be considered to reach a final diagnosis.
