Metastatic esophageal cancer

Diagnosis almost certain

Presentation

Dysphagia and recent weight loss.

Patient Data

Age: 65 years
Gender: Female

Circumferential increased wall thickness suggestive for tumoral infiltration is present at the distal half of the esophagus. There is no sign of local invasion and no mediastinal lymphadenopathy.

A 65×50 mm hetero enhancing mass is present at para cardia and upper pre aortic regions most compatible with lymphadenopathy.

Multiple thin walled non-enhanced lesions are seen at the liver less than 18 mm.

Several small cysts are present in the pancreas less than 10 mm.

Multiple cortical cysts are seen at kidneys less than 115 mm in diameters. A few of them show marginal calcifications. There are also several small parapelvic cysts in the left kidney.

Postoperative changes are seen due to sternotomy and CABG.

Case Discussion

Distal esophageal mass; pathology proven adenocarcinoma with para cardia and upper pre aortic lymphadenopathies.

Renal, hepatic, and pancreatic cysts could be suggestive for autosomal dominant polycystic kidney disease.
