Right upper quadrant pain and elevated liver enzymes.
Patient Data
A few adjacent intraluminal polypoid lesions are seen in the gallbladder that contain small foci of calcification. Additionally, asymmetrical wall thickening is noted in the gallbladder neck.
Multiple ill-defined low-enhancing masses are seen in the liver less than 110 mm, inferring metastases. Intra and extrahepatic bile ducts are of normal caliber.
A small volume of free fluid is present in the abdominopelvic spaces.
A few non-enhanced simple cortical cysts are seen in both kidneys.
Post-operative changes are seen in the right inguinal regions.
The prostate gland is enlarged.
Case Discussion
Pathology-proven gallbladder adenocarcinoma with hepatic metastases and mild ascites.
Gallbladder adenocarcinomas may be present as an intraluminal mass, diffuse mural thickening, or mass replacing the gallbladder, probably the result of progression from both prior patterns, which is the most common presentation.
