Presentation
Known infiltrative central cholangiocellular carcinoma with rapid progression and jaundice.
Patient Data
Necrotic, irregular tumor mass of the liver extending into the portal vein (tumor thrombus). Subsequent prominent dilation of the intrahepatic bile ducts. Liver metastases. Large metastatic lymph nodes in the liver hilum. Posterior mediastinal metastatic lymph nodes. Bilateral adrenal diffuse thickening (more on the right side), likely metastatic. Mild abdominal and pelvic ascites. Small sliding hiatus hernia.
NB: Small tumor thrombus extending into the inferior vena cava.
Case Discussion
Infiltrative hilar cholangiocarcinoma is an aggressive malignancy. The presence of peripheral biliary duct dilation and tumor thrombus is more likely than in hepatocellular carcinoma.
