There is a bilateral symmetrical signal abnormality in the putamen, which appears hyperintense on T2W/FLAIR images and shows diffusion restriction on DWI. Few tiny nodular areas of hypo intensity on SWI with the corresponding hyperintensity on Phase images, suggesting hemorrhage.

Slight increase in the enhancement of the right optic nerve, suggestive of optic neuritis.

Case Discussion

In a given history of altered sensorium and vision problems after alcohol intake, the above imaging findings are consistent with methanol poisoning. Patients usually recover from this condition but he had a persistent vision problem.
