Middle cerebral stroke with hemorrhagic conversion

Presentation

Acutely worsening of mental status following left middle cerebral stroke.

Patient Data

Age: 70 years
Gender: Male

CT at presentation

Hypodensities as well as loss of grey-white differentiation are seen in the left thalamus and left MCA territory concerning for acute ischemic infarct. There is effacement of the anterior horn of the left lateral ventricle. There are two thrombi noted within the left MCA.

CT five days later

ct

CT head obtained 5 days following initial CTA.

Left MCA territory infarction involving the left basal ganglia, temporal lobe, and parieto-occipital junction. A new hyperdensity in the left basal ganglia suggestive of new hemorrhagic conversion.

Case Discussion

This is a stroke within the left middle cerebral artery territory with hemorrhagic conversion.

The patient had a PEG tube placed due to persistent dysphagia and difficulty with oral intake. He was discharged to a nursing facility.
