Diagnosis almost certain

Presentation

Shoulder pain with activity

Patient Data

Age: 45 years
Gender: Female

Mild distension of the proximal long head of biceps tendon sheath, which is nontender on sonopalpation and taken to be within normal limits. The supraspinatus tendon is mildly tendinopathic, with an interstitial tear anterior and distally, without extension to a surface. Bursal surface fraying of the posterior supraspinatus tendon.

Video imaging of the supraspinatus from anterior to posterior in a longitudinal plane.

Video imaging of infraspinatus from medial to lateral in the transverse plane.

Case Discussion

Finding the proximal long head of biceps tendon as a landmark helps orientation with shoulder imaging interpretation. Tendinopathy can be graded as mild, moderate, and severe.
