Distended gallbladder with multiple stones, the largest is seen at the gallbladder neck compressing the corresponding aspect of the common hepatic duct with consequent intra hepatic biliary dilatation. The CBD is not dilated.
Case Discussion
Mirizzi syndrome refers to extrinsic compression of an extra-hepatic biliary duct from one or more calculi within the cystic duct or gallbladder. It is a functional hepatic syndrome but can often present with biliary duct dilatation and can mimic other hepatobiliary pathologies such as cholangiocarcinoma.
