MISME syndrome (NF2)

Case contributed by Dr Hani Salam

Presentation

Known case of NF 2.

Patient Data

Age: 45
Gender: Female
MRI

MRI of the brain shows bilateral internal auditory canals and CPA cistern enhancing masses consistent with vestibulo-cochlea schwannomas. There are multiple extra-axial masses seen in the frontal, parietal and temporal regions consistent with meningiomas. Multiple similar small masses also seen in the posterior fossa.

There is moderately large expansile mass seen in the cervical spinal cord extending superiorly from the medulla oblongata extending down to the level C7. This showed solid enhancing component and cystic changes.

The constellation of findings are consistent with MISME syndrome

Case Discussion

MRI of the brain showed bilateral internal auditory canals and CPA cistern enhancing masses consistent with vestibulo-cochlea schwannomas. There are multiple extra-axial masses seen in the frontal, parietal and temporal regions consistent with meningiomas, which are pathologically proven. Multiple similar small masses also seen in the posterior fossa.

There is moderately large expansile mass seen in the cervical spinal cord extending superiorly from the medulla oblongata extending down to the level C7. This showed solid enhancing component and cystic changes, which is pathologically proven ependymoma.

The constellation of findings are consistent with MISME syndrome
