Mondor disease - penis

Diagnosis almost certain

Presentation

Painful swelling on the dorsum of the penis with bluish discoloration of the related skin.

Patient Data

Age: 50 years
Gender: Male
ultrasound

The dorsal penile vein is seen distended incompressible over a hypoechoic thrombus. No flow could be detected on power Doppler. Moderate subcutaneous edema is noted more so on the dorsal side of the penile shaft. Cavernosal artery was checked for flow and was seen patent with good flow on color/spectral Doppler.

These findings are consistent with superficial thrombophlebitis of the dorsal penile vein or Mondor disease.

Case Discussion

Although superficial thrombophlebitis of the dorsal penile vein is rare, it can be seen in clinical practice. The patient usually presents with a painful swelling on the dorsum of the penis. Predisposing factors include sexual intercourse, coagulopathy, and IV drug use. This is usually a self-limited disease and resolves with appropriate treatment within 4-6 weeks.
