A displaced and overlapped fracture of the ulnar shaft is present. Additionally the radial head is dislocated anteriorly.

A: In addition to the ulnar shaft fracture, the radial head is anteriorly dislocated.

A: The radial head determines the type: type I : classic (originally described by Monteggia) has anterior dislocation of radial head (this case); type II has posterior dislocation of radial head; type III has lateral dislocation of radial head; and type IV has a fracture of radial head as well as ulnar shaft. As is usually the case, in everyday practice describing the fracture / dislocation is far more important than remembering the grade.
