Morel-Lavallée lesion - knee

Case contributed by Dr Mostafa El-Feky

Presentation

Left calf swelling.

Patient Data

Age: 13 years
Gender: Male
MRI

Both knees

A well-circumscribed subcutaneous area of low signal intensity on T1and high signal intensity on T2 and T2 fat sat sequences, noted just superficial to the proximal portion of the left medial head gastrocnemius muscle, containing small lobules of internal fat signal, mural thickening and internal septations.

Case Discussion

In view of patient’s history of trauma, MRI features are in keeping with organized hematoma. The location of the lesion at the subcutaneous tissues just superficial to the muscle fascia makes Morel-Lavallée lesion the likely diagnosis.

Although Morel Lavallée lesion is known for hematoma / degloving injury occurring over the greater trochanter, similar lesion can occur around the knee joint due to similar mechanism.
