Frontal radiograph of chest shows a lobulated radio-opacity in the right paracardaic region silhouetting the right cardiac border likely suggesting its position to be anterior.
Differentials include cardiophernic fat pad, Morgagni's hernia, pericardial cyst, lymphadenopathy and focal diaphragmatic eventration.

There is a focal defect in the anteromedial diaphragm with herniation of omentum into the right thoracic cavity with minimal collapse of the underlying lung.
