Morton's neuroma

Case contributed by Akos Jaray
Diagnosis certain

Presentation

Burning sensation between 3rd and 4th metatarsal heads on the right when walking.

Patient Data

Age: 45 years
Gender: Female
ultrasound

There is a small, protruding, raindrop-shaped hypoechoic lesion between the 3rd and 4th metatarsal heads for lateral compression measuring 6 mm in transverse diameter in keeping with a typical Morton's neuroma. There is no adjacent fluid collection or significant soft tissue swelling.
No significant discomfort for lateral compression.

Case Discussion

The localization of the symptoms was typical for a Morton's neuroma, but the lack of discomfort for lateral compression is fairly atypical.

The dynamic ultrasound examination helps to visualize the hidden lesions by lateral compression.

Feeling a small pop during the maneuver is also reassuring.
