Diagnosis almost certain

Presentation

Dysmenorrhea in a patient with primary infertility.

Patient Data

Age: 35 years
Gender: Female

The right uterine horn is not communicating with the uterine cavity and contains endometrium of high signal intensity on T1WI and T2WI, indicating a functional non-communicating horn. Left unicornuate uterus.

Both ovaries show multiple small follicles.

Minimal effusion is seen in Douglas pouch.

MRI features suggestive of a Mullerian duct anomaly class IIb: unicornuate uterus with a non-communicating contralateral rudimentary horn, containing endometrium.
