Multifocal glioblastoma

Headache and unsteadiness.

Patient Data

Age: Elderly
Gender: Male
Modality: MRI

MRI through the brain demonstrate extensive signal change involving the right cerebellar hemisphere, pons and corona radiata. The T2 white matter hyperintensity is in continuity, with focal regions of haemorrhage (see gradient echo sequence) and enhancement. 

Modality: Pathology

Non-pallisaded necrosis in astrocytoma (WHO grade 4).

Case Discussion

The differential is between a multifocal GBM or metastases. The presence of pontine lesion and the location of the lesions deep within the white matter favours the former. 

This was confirmed on pathology.
