Presentation

Headaches refractory to treatment, history of arterial hypertension and type II diabetes.

Patient Data

Age: 60 years
Gender: Female

A cluster of rounded lesions with high signal on T2, without suppression on FLAIR, is located in the subcortical white matter in the posterior insula and inferior frontal gyrus on the right. This lesion does not enhance after intravenous contrast administration.

Features characteristic of a multinodular and vacuolating neuronal tumor (MVNT).

Case Discussion

A 60-year-old female presented with a severe headache refractory to treatment.

MRI showed a right frontal lesion consisting of a cluster of small nodules, hyperintense on T2WI and FLAIR, not enhancing after gadolinium administration.

The findings described are consistent with a multinodular and vacuolating neuronal tumor (MVNT) but are almost certainly incidental and not the cause of symptoms.
