Presentation

Long history of temporal lobe epilepsy.

Patient Data

Age: 40 years
Gender: Male

In the subcortical white matter of the superior temporal gyrus on the right and within the temporal stem are numerous small nodular/cystic high T2 signal lesions that form a coalescent region without significant mass effect. They do not attenuate on FLAIR. There is no diffusion restriction and no evidence of calcification or blood product.

The remainder of the scan is unremarkable. The hippocampi and mesial temporal lobes are otherwise normal. No evidence of focal lesions elsewhere.

Case Discussion

Multinodular and vacuolating neuronal tumors (MVNT) are "no-touch lesions". Their characteristic appearance makes a radiological diagnosis possible. They are most commonly found in the setting of epilepsy and are considered one of the long-term epilepsy-associated tumors (LEATS).
