Multiple bilateral parotid cysts

Case contributed by Dr Maxime St-Amant

Bilateral parotid glands enlargement.

Patient Data

Age: 60
Gender: Male

MRI shows multiple bilateral parotid lesions, slightly hyperintense T1 compared to CSF and hyperintense T1, compatible with cysts with highly proteinic content. Post gadolinium phase is not shown, but there was no enhancement.

It is also worth noticing that the submandibular glands are atrophic. The patient wasn't known for Sjögren's syndrome, but this hypothesis was submitted to the treating physician.

HIV must always be excluded with this type of lesions, since lymphoepithelial cysts can have this appearance. Bilateral cystic Warthin tumors are highly improbable.
