Parotid cysts - bilateral

Case contributed by Maxime St-Amant
Diagnosis almost certain

Presentation

Bilateral parotid glands enlargement.

Patient Data

Age: 60 years
Gender: Male

Neck

mri

MRI shows multiple bilateral parotid lesions, slightly hyperintense on T1 compared to CSF and hyperintense T2, compatible with cysts with highly proteinic content. Postcontrast sequence is not shown, but there was no enhancement.

Case Discussion

It is also worth noticing that the submandibular glands are atrophic. The patient wasn't known for Sjögren's syndrome, but this hypothesis was submitted to the treating physician.

HIV must always be excluded with these types of lesions, since lymphoepithelial cysts can have this appearance. Bilateral cystic Warthin tumors are highly improbable.
